Q. How do I cite in APA style a Google Forms survey created by me?

How do I cite in APA style a Google Forms survey created by me? I created a Google Forms survey  and I am writing a paper about it. I am not sure how to cite it in my paper when I am talking about the results.

If you created the survey and you're citing the survey form and the survey form is still available for people to see, then follow the rules for Tests, Scales, and Inventories (Section 10.11) and see Example 82. Both are on p. 340 of the APA Publication Manual, 7th ed.

If you're citing the data set you gathered from the survey form, then I'm not sure you need a reference unless your data set is available for other people to see. For the rules on this, see the introduction to 10.9, Data Sets, on pp. 341-342.

If you include the survey and the data set in your paper as appendices, then you will not need to create references for them. However, you will need to mention them in your paper and to direct readers to them. The rules for appendices are in Section 2.14 on pp. 41-42.
